Approach to Spinal Seeding Metastases

Spinal seeding metastases are among the rare but serious complications of primary central nervous system (CNS) tumors. Although the clinical incidence of spinal metastasis is low in glioblastoma and anaplastic astrocytoma, this rate is higher in autopsy series. Medulloblastomas, particularly in childhood, frequently exhibit leptomeningeal involvement, whereas ependymomas can disseminate throughout the neuraxis via cerebrospinal fluid (CSF). Disruption of the tumor capsule during surgery increases the risk of spinal dissemination. It has been shown that the combination of radiotherapy and chemotherapy positively affects survival. Spinal seeding metastases represent a clinical entity that requires a multidisciplinary approach. Early diagnosis and appropriate treatment in high-risk patients can improve both quality of life and survival time. The aim of this article is to evaluate tumor types that spread through CSF, their dissemination mechanisms, and current treatment approaches in light of the literature.
